US vs UK Medical Billing: Key Differences

By Oliver Hive · Published 27 October 2026 · 2 min read

Billing for healthcare in the United States and the United Kingdom looks similar on the surface, but the funding, payers and rules differ. Practices working in both markets need workflows that adapt by region.

How care is funded

In the US, most care is paid by government programmes such as Medicare and Medicaid or by commercial insurers, with patients paying deductibles and coinsurance. In the UK, NHS care is funded centrally. Private practice is paid by individual patients or by private medical insurers.

Payers and claims

US practices submit claims electronically to many payers, each with its own edits, timely filing limits and appeal rules. UK private practices typically deal with a smaller set of insurers and often use invoice-based processes, with pre-authorisation for many treatments.

Coding standards

US billing relies on CPT and HCPCS for services, ICD-10-CM for diagnoses and CDT for dental. UK providers use ICD-10 for diagnoses, and insurers publish their own procedure schedules for private care.

Pre-authorisation and approvals

Both markets use approvals before treatment. In the US it is called prior authorization and is controlled by payer policy. In the UK private sector, insurers commonly require pre-authorisation for hospital treatment and many procedures.

Data protection

US providers handle protected health information under HIPAA. UK providers must follow UK GDPR and the Data Protection Act. Both require careful access control, but the agreements and rights differ.

Working across both

If you operate in both, keep region-specific workflows and reporting. Our teams cover the United States and the United Kingdom, as well as Canada and the UAE.

Example: a clinic in both markets

A group with sites in London and Texas should not run one workflow. The London site needs insurer pre-authorisation and patient invoices, while the Texas site needs eligibility checks, payer edits and timely filing tracking. Shared reporting keeps leaders informed without forcing identical processes.
